Required documents to be submitted with the application:

-A power of Attorney document (if representative is appointed);

-In case a representative has not been appointed and owner of the trademark is an individual, attach a copy of the identity card or passport;

-In case an representative has not been appointed and owner of the trademark is a company, attach a copy of a declaration issued by the company certifying the surrender request.

By means of this application, I am hereby applying for a notice of full or partial surrender of the goods and services for which the mark is registered, or withdrawal of the Trademark application. I confirm that the data I am supplying in this application is correct. I hereby give my consent to the Commerce Department to process and record the personal information which I am providing in this application and to exchange this information with Public Authorities, Government Departments and any other third parties, as necessary, in order to process this application and as permitted by Maltese Law.

I fully understand that:

-by opting out my application cannot be processed;

-authorised personnel who are processing this application may have access to the information provided in this application for the purpose specified in this application;

-information in this application form will be published by the Commerce Department and is therefore accessible to the public;

-edited information, that would not identify me, may be used for statistical puposes.

It has come to the attention of the Commerce Department that local Trademark applicants are receiving invitations to pay fees that do not originate from the Industrial Property Registrations Directorate within the Commerce Department and which are unrelated to the processing of Trademark applications carried out by this office. If you receive an unsolicited invitation regarding renewal, publication, transfer or any other procedure in connection with your Trademark registration, DO NOT PAY IT until you have checked carefully to see what services are being offered and whether the source is official.
