By means of this Application, I am hereby applying for an extension of the Substitution period.
I confirm that the data supplied in this Form is correct. I hereby give my consent to the Commerce Department to process and record the personal information which I am providing in this Form and to exchange this information with Public Authorities, Government Departments and any other third parties, as necessary, in order to process this Form and as permitted by the Laws of Malta.
I fully understand that:
- by opting out, my Application cannot be processed;
- authorised personnel who are processing this Form may have access to
the information provided in this Form for the purpose specified in this Form;
- edited information that would not identify me, may be used for statistical purposes;
- I am entitled to see the information related to me, should I ask for it in writing;
- for the purposes of the Data Protection Act, the Data Controller is the Director-General, Commerce Department.
Documents Required with this Application
- Medical Certificate in the case of illness, pregnancy or where the principal Licensee is taking care of ailing parents, spouse or children
- Birth Certificate where the principal Licensee (female) has given birth to a child or, in the case of adoption, proof that the principal Licensee had adopted a child
- Proof that the parent, spouse/partner, son or daughter, as the case may be, are registered as a disabled person with the Commission for the Rights of Persons with Disability (CRPD) in the case where the principal Licensee is taking care of a disabled parent, spouse/partner, son or daughter
- In the case of fostering, documentary evidence that the principal Licensee is fostering a child, after having been certified able to fact as a foster father or mother, by the Adoption and Fostering Panel of the Department for Family Welfare
- Where the commercial activity involves the retail of food, an approval by the Environmental Health Authorities is required indicating that the premises to be used as a store may be used for storage (*)
* Provided that where the premises of the Licensee is to be used as a Store and such premises has already been certified by the Environmental Health Authorities, the application may instead be accompanied by a declaration stating the use and certification by the Environmental Health Authorities, and signed by both the licensee and the substitute hawker.
