Application for Registration of a
Co-operative Society
Insert name, age, address and occupation of at least five persons, or of at least two persons duly authorised in this behalf of each society (if the application is made by societies) who desire to form a co-operative society under the Cooperative Societies Act, and hereby apply for registration. If the prospective society has more than five founding members, a separate sheet giving their personal details is to be submitted separately and duly signed.
