to be referred to as ‘the student’, hereby
give my consent to the student to give his personal information and submit the student maintenance grants application form.
As parent/legal guardian, I am hereby obliging myself with all obligations imposed by the same application form, on behalf of the student concerned.
Signature of Parent/Legal Guardian
a. All Government forms are subject to monitoring and assessment by the Better Regulation Unit within OPM, to ensure the implementation of Better Regulation within the Public Sector.
b. The Students’ Maintenance Grants’ Board is hereby informing you that it can make use, retain and share information of personal nature and other data which students have submitted with their application, for:
i. the processing of their application, and
ii. the determination of future eligibility,
iii. for statistical and/or study purposes,
in line with the lawful processing under the General Data Protection Regulation, Article 6 (b).
c. The Students’ Maintenance Grants’ Office reserves the right to request, at determined intervals, document as it may deem necessary.
